Here you will find information regarding your Immigration physical exam visit:
Documents needed for your visit:
Government issued photo I.D. such as passport or driver’s license
Form I-693 (https://www.uscis.gov/i-693) with your name and A-number (if applicable) filled out on each page, Part 1, and first portion of Part 2 completed.
Vaccination records. If you do not have vaccination records, that is okay.
(We have the Tdap vaccine and this can be administered on the day of your exam.)
Your I-693 form will generally be ready within 7-10 business days of obtaining labs.
Cost = $250 (This includes the immigration physical exam + completed I-693 form)
Once completed, we will provide you with the completed I-693 form in a sealed envelope. We will also email you a copy of your completed I-693 form.
*Per CDC, Immigration physical exams CANNOT be conducted via telemedicine. Please schedule an in person visit when scheduling.
